Application Forms

Forms necessary to begin the enrollment process at Grace Valley Christian Academy are available for download.

  1. Preliminary Enrollment Application [pdf] - initiates the admissions process (2 pages).
  2. Statement of Cooperation & Commitment [pdf] - form to be completed and returned with the Preliminary Enrollment Application.

If an applicant is selected for enrollment, the following forms will also be required in addition to the Enrollment Contract:

  1. California School Immunization Record [pdf] - This record is part of the student’s permanent education record as defined in Section 49068 of the Education Code and shall transfer with that record. If transferring from another school, this form will be provided by that school
  2. Report of Health Examination for School Entry [pdf] - To protect the health of children, California law requires a health examination 6 months prior to the start of Kindergarten, or 18 months prior to 1st grade. This form should be completed by the health examiner and submitted to the school.
  3. GVCA Handbook Acknowledgement Form [pdf] - sign and return this form after reading the Handbook.

Other Forms

  1. Field Trip Permission Slip[pdf] - Teachers send home a completed form in preparation for field trips. If the teacher's form is not available, print out this form and fill in the field trip information. Return it SIGNED to the classroom to enable your child to attend the field trip.
  2. Field Trip Driver Information Form[pdf] - Parents driving on field trips must complete this form for each field trip. A copy of the driver's license and insurance card are needed once per school year.
  3. Delegation of Authorization for Medical Care[pdf] - Complete this form to inform the school when someone other than a parent/guardian is caring for a student.
  4. Notice of Medications at School Form(2 pages) [pdf] - Any medication (prescription or over-the-counter) must be brought to the school office along with BOTH PAGES of this form completed. Medications are kept in the school office and administered per the information provided on the forms.
  5. Food Allergy Action Plan(2 pages) [pdf] - Students with severe food allergies must have this form on file in the school office. It must be filled in completely and be kept up-to-date AT ALL TIMES.
